Overview:
SingHealth Community Hospitals (SCH) comprises Bright Vision Community Hospital, Sengkang Community Hospital, and Outram Community Hospital. Being Singapore's only cluster of community hospitals allows us to shape the way we deliver person-centered care to patients.
Our community hospitals are poised to set standards in care, exchange best practices, and strengthen collaborations with our healthcare and community partners.
We provide medical, nursing, and rehabilitation services, with an emphasis on rehabilitation to get the patients well enough to return to the community.
What You'll Do:
Recommend and monitor patient drug therapy and provide drug information.
Identify, resolve, and prevent drug-related problems.
Interact with patients, family, and other health care providers to assure continuity of services and to achieve desired patient outcomes.
ensure the accurate and timely distribution of pharmaceuticals, and provide education to patients, staff, and other health care practitioners.
Working Hours:
- 8:30 am - 6:00 pm (Normal Office Hours)
- 8:30 am - 12:30 pm (Saturday)
Job Requirements:
- Degree/Master Graduate from a School of Pharmacy recognized by the Singapore Pharmacy Council.
- Pre-Reg turning Full registration is welcomed to apply.
- Full registration with the Singapore Pharmacy Council.
- Valid membership with a local professional body/association is preferred where applicable.
- Experience as a Pharmacist in relevant clinical settings and demonstrated competency will be an advantage.
